You've introduced me to this song ( as a Canadian - we " heard " RUSH all the time on radio. I bought a few albums...but not Presto ).
On the video* of Scars that I watched - it was Geddy that impressed. A lot calmer than I recall. He settled into this song. Scars of pleasure scars of pain. Atmosphere changes make them sensitive again.
There's a comment to the video by a keyboardist - at 3:14 Geddy sampling his own voice for the callbacks. Calm. Resigned.

I adore this song for both the music and lyrics. Alex does provide a lot of atmosphere on the track, both in the ringing chords and the jangly riffs that cut through, much like nerve pain can be an unexpected jolt.
It seems to me that Neil crafted subtle comparisons to how pleasure and pain may revisit a person, and how societal/national/cultural shifts can cause similar reverberations in observant individuals and groups. Overthinking is my superpower though, so ... 🤷
